COVID-19 Vaccine Receipts

If you were vaccinated in Ontario

You can view and print your own vaccine receipts on demand at: 


Receipts are available:

  • for first and second doses received in Ontario (once you get them)
  • regardless of where you were vaccinated (for example, at a mass immunization clinic, hospital, pharmacy or primary care setting)

To log in and get your receipt, you will need:

  • a green photo health (OHIP) card(you will need numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth
  • your postal code (the one associated with your health card)

If you were vaccinated outside Ontario


If you are a resident of Ontario and received a dose or complete series of the COVID-19 vaccine out of Province or Canada, the province of Ontario requires you to report this information to your local Public Health Unit, to assist with either completing your vaccination series and/or validating your vaccination record in the Provincial COVID-19 reporting system (COVAXon).

Due to the high volume of submissions to Algoma Public Health requests can take up to 10-14 business days.


NOTE: The Ontario COVID vaccine documentation system (COVAXon) is not currently connected to the ArriveCAN application or any other travel-related system and does not replace the need to retain original documentation of COVID vaccine for travel-related purposes. You must have an Ontario residence, and more specifically, a residence (address) within the Algoma district, to submit your proof of vaccine to Algoma Public Health to be entered into COVax and receive an Ontario Vaccine Receipt. If you will be moving to Algoma in the future, please wait until you arrive to submit your documents. 


An Ontario Health Card is not required to submit proof of “out of province” doses. Persons with valid Ontario Health card will be able to access to download proof of vaccination following their vaccine submission and approval.
