Personal Service Setting Notification Form

If you offer a personal service within the district of Algoma, the regulation requires you to notify Algoma Public Health whenever you are:

  • Opening a new business,
  • Renovating an existing business, or
  • Offering new services within an existing business.

You must notify APH at least 14 days before the planned opening, renovation, or addition of new services. To notify Algoma Public Health, please provide the following information by filling out the form below, or by contacting the Environmental Health Department by phone at (705) 759-5286:
