View & Submit Immunizations
Information is available for parents or guardians who received a letter from APH about updating student immunizations.
You can view your current immunization record or report any new immunizations from healthcare providers.
There are two ways you can update your immunization record, through the provincial reporting tool or directly with APH.
Update your record with the Provincial reporting tool:
Immunization Connect ONtario (ICON) is an online service provided by the Ministry of Health and Long-Term Care that allows you to submit or view your immunization information.
You will need your Health Card and postal code (you may be asked to create an account with a 6-digit pin) or use your Ontario Immunization Identification (OIID).
You will also need to upload a copy of your immunization record. If a copy of your immunization record is not submitted, the vaccines will not be uploaded into the system. Please reach out to the Immunization Phone line: 705-759-5409 for further information.
Please note: Your immunization submission can take up to 5 business days to show up in ICON
Update your record directly with APH:
- Bring in a copy of the immunization record to any of our locations during regular business hours
- Call your local APH office
- Fax a copy of the immunization record to 705-541-5959
- Mail a copy of your immunization record to:

- Email a copy of your immunization record, including full name and date of birth to records@algomapublichealth.com
Please note: If you are sending us personal information by email, please be aware that electronic communication is not always secure and can be vulnerable to interception.

Request your immunization record directly with APH:
- Call our Records department to request a copy at 705-942-4646 ext. 3535.
- Complete a request for immunizations records form at your local APH office
Please note: It may take up to 10 business days to receive your immunization record if you make a request by phone or in person.
You can view and print your COVID-19 vaccine receipts. If you received a COVID-19 immunization outside of Ontario, please submit proof of your vaccination to records@algomapublichealth.com or contact us at our confidential Immunization Records line at 705-942-4646 ext. 3535
