This form is for local non-profit community agencies, programs and services (e.g. schools, post-secondary institutions, libraries, food security programs, shelters, etc.) to order condoms, dental dams, and lubricant from Algoma Public Health

If you are looking for condoms for yourself/your partner, you can drop by the office nearest you to pick up free condoms and lube.

Condom Storage Tips:

• Always check the expiry date on the package; properly stored condoms can be used until this date.
• Store condoms at room temperature and away from direct sunlight. Improper storage may cause them to age or deteriorate faster.
