48-Hour Postpartum Follow-Up
As part of the HBHC program, we offer follow-up after a baby is born. All parents who consent while in the hospital after the birth of their baby will be contacted by a Public Health Nurse via phone or text message within 48 hours of discharge. The Public Health Nurse will complete a quick phone assessment, provide support and answer questions about caring for the baby, infant feeding, and recovery from birth. In some cases, a home visit will be arranged for additional support.
